This position reports to the clinical support manager and will provide any of the following: project management; planning; application development and support; hospital information systems; installation, support, and maintenance of systems and computing or network equipment; direction to analyst positions, end user interaction and consulting. Bachelors Degree in Physical Therapy from an accredited program. Licensed or eligible for licensure in Louisiana to practice Physical Therapy.
Provides quality patient care including the performance of evaluations, development of treatment/discharge plans and the implementation of therapy in respective disciplines.

The RN Case Manager functions in the role of a nurse home visitor in the nurse home visiting project. The Case Manager is responsible for coordinating, negotiating, procuring, and managing the care of complex patients to facilitate achievement of quality and cost outcomes. Works collaboratively with interdisciplinary staff, internal and external to the organization. Participates in the development of care plans to include short and long term goals related to clinical, psychosocial, and discharge planning issues. All cases of perinatal/infant mortality will be reviewed by the Case Manager.
Physician Assistant degree required with Louisiana state licensure eligibility. Doctorate degree preferred.
Job duties are flexible and may include: classroom teaching, problem-based learning group facilitation, mentoring students, coordinating courses, applicant interviews, research and PA Organization service. Primary care clinical experience preferred. Salary is commensurate with experience. Academic appointment is based on academic degree, evidence of research, service, and teaching excellence. PA`s must be certified by the NCCPA.
